    Biological treatment of the knee with platelet-rich plasma or bone marrow aspirate concentrates

    ABSTRACT — Knee pathologies including focal cartilage injuries, osteoarthritis (OA), and ligament injuries are common. The poor regeneration and healing potential of cartilage has led to the search for other treatment modalities with improved healing capacity. Furthermore, with an increasing elderly population that desires to remain active, the burden of knee pathologies is expected to increase. Increased sports participation and the desire to return to activities faster is also demanding more effective and minimally invasive treatment options. Thus, the use of biologic agents in the treatment of knee pathologies has emerged as a potential option. Despite the increasing use of biologic agents for knee pathology, there are conflicting results on the efficacy of these products. Furthermore, strong data supporting the optimal preparation methods and composition for widely used biologic agents, such as platelet-rich plasma (PRP) and bone marrow aspirate concentrate (BMAC), largely remain absent from the literature.     A four-season prospective study of muscle strain reoccurrences in a professional football club

    The aim of this investigation was to characterise muscle strain reinjuries and examine their impact on playing resources in a professional football club. Muscle strains and reoccurrences were prospectively diagnosed over four seasons in first-team players (n = 46). Altogether, 188 muscle strains were diagnosed with 44 (23.4%) of these classed as reinjuries, leading to an incidence of 1.32 strain reoccurrences per 1,000 hours exposure (95% Confidence Interval [CI], 0.93–1.71). The incidence of recurrent strains was higher in match-play compared with training (4.51, 95% CI, 2.30–6.72 vs 0.94, 95% CI, 0.59–1.29). Altogether, 50.0% of players sustained at least 1 reoccurrence of a muscle strain, leading to approximately 3 days lost and 0.4 matches missed per player per season. The incidence of recurrent strains was highest in centre-forwards (2.15, 95% CI, 1.06–3.24), peaked in May (3.78, 95% CI, 0.47–7.09), and mostly affected the hamstrings (38.6% of all reoccurrences). Mean layoff for nonreoccurrences and recurrences was similar: ∌7.5 days. These results provide greater insight into the extent of the problem of recurrent muscle strains in professional football

    Introduction: anthropology's queer sensibilities

    This special issue addresses vital epistemological, methodological, ethical and political issues at the intersections of queer theory and anthropology as they speak to the study of sexual and gender diversity in the contemporary world. The special issue centres on explorations of anthropology’s queer sensibilities, that is, experimental thinking in ethnographically informed investigations of gender and sexual difference, and related connections, disjunctures and tensions in their situated and abstract dimensions. The articles consider the possibilities and challenges of anthropology’s queer sensibilities that anthropologise queer theory whilst queering anthropology in ethnographically informed analyses. Contributors focus on anthropologising queer theory in research on same-sex desire in the Congo; LGBT migrant and asylum experience in the UK and France; same-sex intimacies within opposite gender oriented sexualities in Kenya and Ghana; secret and ambiguous intimacies and sensibilities beyond an identifiable ‘queer subject’ of rights and recognition in India; migrant imaginings of home in Indonesian lesbian relationships in Hong Kong; and cross-generational perspectives on ‘coming out’ in Taiwan and their implications for theories of kinship and relatedness.     Electro-thermal impedance spectroscopy applied to an open-cathode polymer electrolyte fuel cell

    The development of in-situ diagnostic techniques is critical to ensure safe and effective operation of polymer electrolyte fuel cell systems. Infrared thermal imaging is an established technique which has been extensively applied to fuel cells; however, the technique is limited to measuring surface temperatures and is prone to errors arising from emissivity variations and reflections. Here we demonstrate that electro-thermal impedance spectroscopy can be applied to enhance infrared thermal imaging and mitigate its limitations. An open-cathode polymer electrolyte fuel cell is used as a case study. The technique operates by imposing a periodic electrical stimulus to the fuel cell and measuring the consequent surface temperature response (phase and amplitude). In this way, the location of heat generation from within the component can be determined and the thermal conduction properties of the materials and structure between the point of heat generation and the point of measurement can be determined. By selectively ‘locking-in’ to a suitable modulation frequency, spatially resolved images of the relative amplitude between the current stimulus and temperature can be generated that provide complementary information to conventional temporal domain thermograms

    "It is her responsibility": partner involvement in prevention of mother to child transmission of HIV programmes, northern Tanzania

    <p>Abstract</p> <p>Background</p> <p>Partner involvement has been deemed fundamental in prevention of mother to child transmission (PMTCT) programmes, but is difficult to achieve. This study aimed to explore acceptability of the PMTCT programme components and to identify structural and cultural challenges to male involvement.</p> <p>Methods</p> <p>The study was conducted during 2007-2008 in rural and urban areas of Moshi in the Kilimanjaro region of Tanzania. Mixed methods were used, and included focus group discussions with fathers and mothers, in-depth interviews with fathers, mothers and health personnel, and a survey of 426 mothers bringing their four-week-old infants for immunization at five reproductive and child health clinics.</p> <p>Results</p> <p>Routine testing for HIV of women at the antenatal clinic was highly acceptable and appreciated by men, while other programme components, notably partner testing, condom use and the infant feeding recommendations, were met with continued resistance. Very few men joined their wives for testing and thus missed out on PMTCT counselling. The main barriers reported were that women did not have the authority to request their husbands to test for HIV and that the arena for testing, the antenatal clinic, was defined as a typical female domain where men were out of place.</p> <p>Conclusions</p> <p>Deep-seated ideas about gender roles and hierarchy are major obstacles to male participation in the PMTCT programme. Empowering women remains a huge challenge. Empowering men to participate by creating a space within the PMTCT programme that is male friendly should be feasible and should be highly prioritized for the PMTCT programme to achieve its potential.</p

    Nitrogen Blanketing and Hydrogen Starvation in Dead-Ended-Anode Polymer Electrolyte Fuel Cells Revealed by Hydro-Electro-Thermal Analysis

    Dead-ended anode operation has a number of practical advantages that simplify system complexity and lower cost for polymer electrolyte fuel cells. However, dead-ended mode leads to performance loss over time which can only be reversed by performing intermittent purge events. This work applies a combined hydro-electro-thermal analysis to an air-cooled open-cathode fuel cell, presenting experimental functional maps of water distribution, current density and temperature. This approach has allowed the identification of a 'nitrogen blanketing' effect due to nitrogen cross-over from the cathode and a 'bypass' effect where a peripheral gap between the gasket and the GDL offers a hydrogen flow 'short circuit' to the border of the electrode. A consequence of high local current density at the margin of the electrode, and resulting high temperatures, may impact the lifetime of the cell in dead-end mode

    A Hypergraph Dictatorship Test with Perfect Completeness

    A hypergraph dictatorship test is first introduced by Samorodnitsky and Trevisan and serves as a key component in their unique games based \PCP construction. Such a test has oracle access to a collection of functions and determines whether all the functions are the same dictatorship, or all their low degree influences are o(1).o(1). Their test makes q≄3q\geq3 queries and has amortized query complexity 1+O(log⁥qq)1+O(\frac{\log q}{q}) but has an inherent loss of perfect completeness. In this paper we give an adaptive hypergraph dictatorship test that achieves both perfect completeness and amortized query complexity 1+O(log⁥qq)1+O(\frac{\log q}{q}).Comment: Some minor correction

    ‘As soon as they can hold a glass, they begin taking alcohol’: a qualitative study on early childhood substance use in Mbale District, Uganda

    Background Globally, substance use is a leading contributor to the burden of disease among young people, with far reaching social, economic and health effects. Following a finding of harmful alcohol use among 5-8-year-old children in Mbale District, Uganda, this study aims to investigate community members’ views on early childhood substance use among children below the age of 10 years. Methods In 2016, we conducted eight focus group discussions with 48 parents and 26 key informant interviews with teachers, health workers, alcohol distributors, traditional healers, religious leaders, community leaders and youth workers. We used thematic content analysis. Four participants and two research assistants reviewed and confirmed the findings. Results Alcohol in everyday life: ‘Even children on laps taste alcohol’: Almost all participants confirmed the existence of and concern for substance use before age 10. They described a context where substance use was widespread in the community, especially intake of local alcoholic brews. Children would access substances in the home or buy it themselves. Those living in poor neighbourhoods or slums and children of brewers were described as particularly exposed. Using substances to cope: ‘We don’t want them to drink’: Participants explained that some used substances to cope with a lack of food and resources for childcare, as well as traumatic experiences. This made children in deprived families and street-connected children especially vulnerable to substance use. Participants believed this was a result of seeing no alternative solution. Conclusions To our knowledge, this is the first study to describe the context and conditions of childhood substance use before age 10 in Mbale District, Uganda. The study shows that community members attributed early childhood substance use to a social context of widespread use in the community, which was exacerbated by conditions of material and emotional deprivation. These social determinants for this practice deserve public health attention and intervention.publishedVersio
